Essential Job Functions: **This job description reflects assignment of essential job functions and does not prescribe or restrict the tasks that may be assigned**
**Position is subject to Grant funding**

1. Works with faculty and other providers and auxiliaries on management of patient oral health issues.
2. Collects and input data related to patient satisfaction surveys.
3. Conducts data entry and generating data from multiple clinic sites for grant related reports.
4. Consults with patients' caregivers, healthcare providers and social services providers to facilitate access and utilization of services.
5. Schedules patient appointments, facilitates transportation, insurance information and medical record needs.
6. Responds appropriately to the concerns of patients, outside agencies and other third-party personnel.
7. Distributes and retrieves diagnostic data (prescriptions, lab tests and blood test results).
8. Remains apprised of activities, current and planned, with the programs.
9. Greets patients and visitors in a courteous, professional manner.
10. Checks for completeness of record and input all information into computer.
11. Answers phone calls in an accurate and timely manner, while being professional and courteous. Maintain message log.
12. Conducts follow up phone calls for cancelled and missed appointments.
13. Recruits patients for grant approved programs.
14. Assists with residents to supply data for needed projects related to the grant.
15. Contacts programs' director(s) of any changes to the residents’ schedules at grant clinics (i.e. school, clinic, University closures).
16. Monitors doctor's treatment plans in order to schedule follow-up appointments.
17. Performs necessary office and clerical functions.
18. Completes other projects as required.
19. Performs other duties as required or assigned.
Marginal Job Functions: 1. Answers routine questions from residents, patients, faculty and staff and refer them to appropriate personal when unable to assist them.
2. Scans Axium documents into appropriate tabs on a daily basis.


Required Knowledge, Skills, & Abilities: 1. Ability to perform many tasks in a relatively short time frame and with established deadlines.
2. Ability to travel between locations.
3. Ability to analyze facts, exercise judgment and arrive at valid conclusions.
4. Proficient in MS Office (Word, Excel, etc.) and ability to learn new software systems.
5. Professional, organized, detailed-oriented.
6. Ability to maintain confidential information.
7. Strong organizational skills and meticulous record keeping.
8. Exceptional people skill, presentation skills, and phone etiquette skills.
Job Requirements:  
Required Certifications/Licensures:  
Required Education: High School Diploma or Equivalent
Major (if required):  
Required Experience: High School Diploma and four (4) years' experience in a directly related field,

-OR-

Associates Degree or 60 college credits and two (2) years' experience in a directly related field

-OR-

Bachelor's degree and one (1) year experience in a directly related field,
Preferred Qualifications: 1. Experience in Electronic Health Records (EHR) systems, preferably with AxiUm software.

2. Bilingual Spanish - English.
