Primary Purpose:

Developing professional and community relationships with patients with neurological diseases, primarily amyotrophic lateral sclerosis, and their caregivers/family members. The Associate will be responsible for the coordination of ALS care services for patients and the creation of a community outreach program to reach our underserved and underrepresented patients in South Florida. This role involves facilitation and dissemination of resources, patient and caregiver education, and information to help navigate this disease process.

Essential Job Functions: 

• Work with the ALS Clinic leadership team to identify and resolve operational and service barriers for reaching underserved and underrepresented patients with ALS within the South Florida area.
• Conduct specialized care consultations and needs assessment for people living with ALS caregivers, and families in a variety of settings including virtual visits, by phone/email, in clinic, at support groups and at home visits, when appropriate.
• Actively manage a caseload of patients with ALS and ensure needs are met
• Work with the providers with the multidisciplinary clinic team and maintain strong communication with clinic staff
• Provides in-service education programs to clinic staff, patients and caregivers, local hospice, home health agencies, hospitals, skilled care facilities and other state social service agencies as needed.

4. Travel - Must be able to travel to/from clinic setting multiple days per week
5. May be exposed to short, intermittent, and/or prolonged periods of sitting and/or standing in performance of job duties.
6. May be required to accomplish job duties using various types of equipment/supplies, to include but not limited to pens, pencils, and computer keyboards.

Required Certifications/Licensures: 
• Supervised related professional experience, including direct care, case management and community resource linkage

Required Education: Bachelor’s degree

Major (if required: Human service-related field (social work, sociology, psychology, human services, public health

Required Experience: • Supervised experience in case management
• Excellent organizational skills and attention to detail
• Display strong work ethic, personal integrity, empathy.
• Ability to work independently with little supervision; must be a self-starter and demonstrate skills to initiate projects independently
• Ability to manage multiple projects simultaneously
• Ability to travel locally
• Proficiency with Excel, Word, PowerPoint, Zoom, etc.

Preferred Qualifications: 

1. Experience in medical setting or healthcare related duties.
2. Bilingual in Spanish and English (preferred, but no required)
